  • Please bring your ID and insurance card to your appointment.

    State of Louisiana COVID-19 Vaccine Eligibility: All individuals aged 5 and up for Pfizer vaccine. At this time, Arbor is only offering the Pfizer vaccine.

***Although the vaccine itself is free, there is a small administration fee we have to charge. This fee will be either billed to the patient’s private insurance company OR billed to the uninsured fund. The patient will not be responsible for any copay, deductible, or fee.
