IMPORTANT APPOINTMENT INFORMATION:

  • Please make an appointment by filling out the form below.

  • We are not accepting walk-ins at this time.

  • Please bring your ID and insurance card to your appointment.

  • Although the vaccine itself is free, there is a small administration fee we have to charge. This fee will be either billed to the patient’s private insurance company OR billed to the uninsured fund. The patient will not be responsible for any copay, deductible, or fee.


COVID-19 Vaccine BOOSTER Eligibility:

Pfizer: If you're over 12 years old and at least 5 months out from your 2-dose Pfizer series, you are eligible for a booster.

Moderna: If you're over 18 years old and at least 5 months out from your 2-dose Moderna series, you are eligible for a booster.

Johnson & Johnson: If you're over 18 years old and 2 months out from your one shot Johnson & Johnson vaccination, you are eligible for a booster.

People should talk to their healthcare provider about their medical condition, and whether getting an additional dose is appropriate for them.

Prescription or notation from a physician or other prescriber is not necessary at this time.

Third Dose vs. Booster

If you are severely immunocompromised, a third dose of the Moderna or Pfizer vaccine is available to you one month after your second shot.

Call us at 225-618-7150 to schedule your appointment for your third dose, if you qualify or have any questions about qualification. You DO NOT need to fill out the form below.

Third dose criteria: For those who are severely immunocompromised:

People who are moderately to severely immunocompromised are especially vulnerable to COVID-19 because they are more at risk of serious, prolonged illness and may benefit from an additional dose to make sure they have enough protection against COVID-19.

Moderately to Severely Immunocompromised includes people who have:

  • Been receiving active cancer treatment for tumors or cancers of the blood

  • Received an organ transplant and are taking medicine to suppress the immune system

  • Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system

  • Have moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)

  • Have an advanced or untreated HIV infection

  • Are currently receiving active treatment with high-dose corticosteroids or other drugs that may suppress your immune response

  • Have other conditions which cause moderate or severe immunosuppression similar to the above conditions
